Welcome to MTI Physical Therapy!

We are happy that you chose MTI Physical Therapy. We are looking forward to welcoming you to one of our seven physical therapy clinics located in Bellevue, Edmonds, Fremont, Magnolia or First Hill.

Your highly trained physical therapist will work with you to identify the cause of your pain or dysfunction and will put together a customized treatment plan tailored just for you. We are looking forward to helping you to get back to the activities you love, pain-free!

Below is a link to a packet of forms that you will need to fill out prior to being seen. If you print them and complete them prior to your appointment, this will make the check-in process quicker. You can also fill them out online using Adobe Acrobat Reader and email them back to us. We always recommend that new patients arrive 15 minutes prior to your first appointment either way. Insurance & Billing Info

At MTI Physical Therapy, we accept a wide variety of insurance plans.  As a courtesy to our patients, we will verify your health benefits prior to your arrival to determine if there will be a copay, deductible, or coinsurance for physical therapy services.

We can provide physical therapy services directly without referral. Many insurance companies, however, require a physician referral or prescription to pay for services.

Please check with your insurance company to determine your PT benefits, deductible, co-pays and/or if a prescription is required.

MTI's Payment Roadmap

WHY DOES MTI COLLECT PARTIAL PAYMENT AT TIME OF SERVICE?

Most insurance plans have deductibles that reset every year on January 1st. It is our company’s policy to collect payment towards your deductible during each visit until it is fully met. The deductible amount is set by your insurance company and is based on the plan that you or your employer chooses.

WHAT IS MTI’S PAYMENT ROADMAP?

In order to provide you with the best possible experience at our clinics, we want to be clear about MTI’s Payment Roadmap, which includes a Point-Of-Service (POS) collections policy:

  • We collect a down payment of $75 during each visit, which will go towards your visit balance and be applied directly to your plan’s deductible.
  • Any additional amount determined by your plan will be billed to you after your insurance company has processed your visit.
  • Once your deductible has been met, the down payment rate will change to between $5-50 depending on your plan’s co-insurance rate.
  • Any overpayments will be refunded to you after you’ve completed treatment and all visits have been processed by your insurance company.

UHC Contract termination

All of our patients are very important to us and we keep our patients’ best interests in mind with every decision we make. In order to continue to provide the highest level of one-to-one care to our patients, we had to make the difficult decision to terminate our contract with United Healthcare (UHC) as of December 1, 2023. The following plans will be affected by this contact termination: United Healthcare (UHC), UMR, ANC group (United Healthcare – Definity), Optum Health, UHC Medicare Advantage PPO and HMO, UHC West, AARP Medicare Complete, All Savers, Oxford, Medica, Surest and Secure Horizons.

As of September 11, 2023, MTI Physical Therapy (MTI) will no longer be accepting new patients who are United Healthcare members.

For existing UHC patients, we will no longer be able to submit claims for in-network coverage after November 30, 2023. We hope that you will be able to complete your Plan of Care and achieve your goals before that time.

If you are an existing patient who wishes to continue care after November 30th, 2023, you will be converted to a “fee for service” payment system and then have the option of personally seeking reimbursement from UHC if you have “out of network” benefits.

I want to come prepared for my first appointment – what should I do?

Please have your prescription and insurance card available when you call. This will allow us to verify your insurance benefits and provide you with an estimate of coverage.

There are lots of physical therapy companies – why should I choose MTI?

As a patient you have the right to receive physical therapy at any place you choose. At MTI Physical Therapy, you will receive the highest quality care while achieving the results you need. This is because all of our physical therapists have advanced training in orthopedic manual physical therapy or sports therapy. You will never be treated by an aid or assistant - we only employ licensed physical therapists.

We know that you are as unique as your fingerprint and work diligently to assess what is causing the pain in your body and develop a personalized plan to address it. This plan will include the use of manual therapy and other physical therapy techniques, specifically dosed exercises tailored for you, and education so that you understand what’s happening in your body. The goal is not only to treat your current dysfunction, but to also help you learn how to avoid future issues in your body by truly understanding the source of your pain. We will partner with you and your physician every step of the way so that you can get back to doing the activities you love.

What should I know about physical therapy (PT)?

Physical therapy is a medical, hands-on approach to relieving pain and restoring function. Physical therapists are medical experts in musculoskeletal and neurological conditions and work closely with physicians. 

Our physical therapists evaluate your movement patterns, identify the cause of your pain, and work to eliminate it.

While every patient’s condition and pain is unique, our physical therapist will use a variety of gentle hands-on techniques, exercises, and equipment to relieve pain quickly and restore your function to optimum levels. 

The overall goal is to restore pain-free mobility and function and to prevent the injury or pain from recurring.

Do you treat my condition at your clinic?

Physical therapists have years of university level training in anatomy, physiology, kinesiology (study of joint movement), neurology, and many of the other medical sciences. 

Our physical therapists have graduated from accredited physical therapy programs and are state licensed. Depending on the university, the physical therapy program is either a masters or doctorate level degree.
